Policies

Arrival time: We ask that you come to your appointment 15 minutes early (for follow-up patients and new patients with all forms filled).  If you are a new patient and have not filled out our forms, please arrive 30 minutes early.

Insurance and Eligibility: Please bring your insurance card to each of your appointment.  You will also need your picture ID card.  As a courtesy to you, our valued patient, we will file the insurance claim on your behalf for payment of service.  However,  any unpaid balance is ultimately the patient’s responsibility.  It is the patient’s responsibility to make certain that his/her insurance coverage is current.

HMO patient: It is your responsibility to make sure Dr. Karin Li is your assigned Primary Care Physician with your Medical Group (IPA).  Any balance unpaid by your insurance/IPA will be your responsibility.  

Payment(s): All payments will be due at the time of check-in.  These payments include co-pays, deductibles, and co-insurance.   We accept cash, credit card, ATM card, and personal check.  There is a $25 charge for “bad checks”.

Prescription Refills: All medication refills will be made during your office visit.  In some cases, Dr. Li may refill certain medications if given at least 1 week’s notice in advance.

Medications that CANNOT be Filled by Phone: These include any medications regulated by the Drug Enforcement Agency, narcotics, pain medications, muscle relaxants, anxiety medications. Patients cannot get more than a 1 month supply and need an office appointment. I also will not be refilling these medications on a long term basis.

Appointment Cancellations: Please call at least 24 hours before your appointment if you plan to cancel, and to avoid a “no-show” charge of $50 or more.

Current Medications: Please bring your medications (e.g. including medication bottles and boxes) to your office appointment.

Patient Feedback: Dr. Karin Li values all feedback from her patients regarding her practice.  Please do not hesitate to talk or write to her about your experience with her or her staff.  All comments will remain strictly confidential with the doctor, unless patient instructs otherwise.
