Note that the first appointment will be for consultation only, and treatment(s) may or may not begin until a later appointment.
It is very important that you complete the following tasks for your appointment with Dr. Karin Li
- Download and fill out the forms below and BRING to your doctor’s visit.
- Bring your Insurance card and Picture ID card.
- All co-pays, deductibles, and co-insurance will be collected during check-in. We accept cash, credit card, check and ATM card. There is a $25 charge for bounced check.
- Bring all your medications (including prescription bottles, boxes).
- Bring all pertinent medical records (previous doctor’s notes), radiology reports, and lab reports/blood work results.

Return Patient (Follow-up)
- Bring your Insurance card and Picture ID card.
- All co-pays, deductibles, and co-insurance will be collected during check-in. We accept cash, credit card, check and ATM card. There is a $25 charge for bounced check.
- Bring your medications (including prescription bottles, boxes).
- Bring any new medical records since your last visit: new doctor’s reports, new radiological reports (ex: X-rays, MRI, CT scan) and new lab reports/blood work results.

Medical Release Form
- This form allows us to release your medical information to another doctor upon your request. It is also for allowing another doctor to send your medical information to our office upon your request. Click on the link below to download the medical release form.
